4. launch game
5. plug in headphones back to USB (windows will switch to the headphones for default playback, game will not)

Do you have any non USB headphones? If so, do you get the same problem with your regular headphones?

I am unable to reproduce this problems and can plug in / unplug headphones repeatedly while the game is running without a problem.

this only happens when the actual "device" in windows is switched.

Plugging in a set of headphones to an existing active device is not causing an issue.

I do have a set of non-usb headphones, but I prefer to use the USB one I have as it also has a microphone in it.

Exact devices (as listed under device manager)
"High Definition Audio Device" (listed twice) - this is the main on-board sound system that the speakers are hooked up to. I use a logitech 5.1 system with this, and yes it does have a standard headphone jack I can use as frequently as I want to without causing a problem.

"Logitech USB Headset" - this one does not show up until the headset is plugged into USB. When this happens, it is configured to become the "default playback device". Unplugging it results in that device removed from device manager list.

Yeah, I can make it happen anytime I stop the sound device, or change it to another one.

Plugging in a headset normally sets them to default from none if you had none in the computer.
If you never change the device (like already having it set to HDMI) then it will not happen. Even plugging in a headset when you already have a sound device doesnt cause it, until you change the device.

default playback device is not automatically adjusted, but for reasons of voice chat etc I have it setup to where plugging it in does change it to the default playback device. Because this is a USB device that sits on a completely separate driver than the original high def audio that I normally use, the switch is usually not recognized by most software. For a game though, it's kind of important cause people stay logged in for a long time and would hate to restart the game just to have sound every time a device is changed. In order to correct this problem, game would have to monitor for any changes in "default playback device" in windows and adjust output to that device accordingly. Even if it means re-initializing sound after each switch.
