Need Information: Tool through chest.

For those that were logged out and reentered the game to find a shovel or tool through their chest:

1) Was this a tool you were previously carrying?

2) What tool was it?

3) Were you carrying anything in the bundle slot (where grass, branches and scrap goes) when you were logged out?

4) Were you able to drag the blank icon from the bundle slot and discard this tool?
