No NPC Cities, EVERYTHING is player made even quests.
You can set your totem EVERYWERE if you don't place it within or close to another tribe territory. Once you placed it you are free to build and terraform if the underground allows it, massiv granit for example can't be terraformed but this is only the case in mountain area.
